How Oraform compares
Two side-by-side pages, and the figures behind them. Both of the tools below are good at a great many things. Neither of them is built only for clinics.
Why a healthcare-only form tool
A general-purpose form builder has to serve a conference signup, a t-shirt order and a new-patient packet with the same product. That works, right up to the point where the thing you are collecting is protected health information — and then the differences stop being about features and start being about which plan you are on.
Both tools on this page can sign a business associate agreement. Neither does it on the plan most clinics would otherwise buy: Jotform puts HIPAA behind its Gold tier, and Cognito Forms puts it behind Enterprise. So the honest price of a general-purpose builder, for a clinic, is not its advertised price. It is the price of the one tier that will sign.
Oraform has one plan. HIPAA hosting and a signed BAA are in it, because there is no version of this product that collects anything other than patient information. That also decides what gets built: save and resume with reminders for the patient who stopped halfway, QR check-in at the front desk and a link, both pointing at the same form, an alert when an answer needs somebody to look at it today, and drop-off figures per question so you can see which one is losing people.

Jotform signs a BAA from Gold up, and Gold is a single-user plan capped at 100 forms and 10,000 submissions a month. Oraform does not meter forms, submissions or staff logins.
