What is different
Which plan signs. Cognito Forms keeps its HIPAA features and its business associate agreement on Enterprise. Oraform signs on the only plan it has, before a single patient answer is collected — so the plan you compare on price is the plan you are actually allowed to use.
What a seat costs. Enterprise includes 20 users and charges $5 a month for each one after that. Oraform charges nothing per user, so adding a receptionist in March does not change the invoice.
What it was built to do. Cognito Forms is a capable general-purpose builder with real strengths in calculations and logic. Oraform only does clinic paperwork, which is why the product has save and resume with patient reminders, QR check-in and link access to the same form, safety alerts on answers, and drop-off figures per question.

Built for clinics
These are not rows in the table above, and that is deliberate: they are things Oraform does, not claims about what Cognito Forms does not. We have only checked Cognito Forms against its own published pricing, which says nothing either way about most of them.
- Save and resume, with reminders. A patient who stops halfway gets an email picking up where they left off, instead of starting the packet again in your waiting room.
- QR check-in, or a link. The same form, reached by scanning a code at the front desk or by following a link sent ahead of the visit.
- Safety alerts on answers. You choose which answers matter — a PHQ-9 item, a score band, a particular yes — and who gets told the moment one comes in.
- Drop-off stats per question. Which question people quit on, so the one quietly costing you patients is a number rather than a hunch.
- Retention set per clinic. How long submissions are kept is yours to set, and it is written into your agreement.
- No per-user fees. Front desk, clinicians and billing all get their own login at no extra cost.
Where Cognito Forms is a better fit
Genuinely. If any of these is what you need, Cognito Forms is the better buy and we would rather you knew now.
- You want to build and change forms yourself, tonight, without asking anyone. It has a no-cost tier and self-serve checkout; Oraform starts with a call.
- You need real arithmetic inside a form — pricing, quantities, conditional totals. Calculations are something Cognito Forms is genuinely good at.
- Most of what you collect is not patient information. Registrations, order forms, internal requests — one account covers all of it, and Oraform covers none of it.
- You want a template library to start from. Theirs is far larger than ours, because we build your forms rather than hand you a catalogue.
- You have 20 staff or fewer and the per-user fee never bites. Then that line in the table above costs you nothing.

Its HIPAA features and a signed business associate agreement sit on the Enterprise plan, according to Cognito Forms' own published pricing page as last checked on October 13, 2026. The tiers below Enterprise do not include them. Oraform includes HIPAA hosting and a signed BAA on its only plan, and signs before any patient information is collected.
Does Cognito Forms charge per user?
Enterprise includes 20 users, and additional users are 5 dollars per user per month on its published pricing. Oraform does not charge per user at all: front desk, clinicians and billing each get their own login at no extra cost.
Can we move our existing Cognito Forms over?
Yes, and we do the work. Send us the forms you use today, in whatever format they are in, and we rebuild them in Oraform with every question and every consent carried over word for word. There is no extra charge for rebuilding forms you already have.
Is Cognito Forms free?
It has a no-cost tier, and for a contact form or an event signup that may be all you ever need. It does not carry the HIPAA features or the business associate agreement, which are on Enterprise, so it is not a fit for patient information. Oraform has no no-cost tier: one plan, and a conversation first, because we build the forms for you.
What does Oraform do that Cognito Forms does not?
The things a clinic-only tool can assume. Save and resume with reminders for the patient who stopped halfway, QR check-in at the front desk and a link, both pointing at the same form, an alert when an answer needs somebody to look at it today, drop-off figures per question, and data retention set per clinic. Cognito Forms is a general-purpose builder and these are not what it was shaped around.
