We rebuild up to ten of the forms you use today — whatever format they're in — carrying over every question and consent word for word, and match them to your branding. It is $499, and free for clinics that get started by November 30.

On monthly billing, yes, once: $79 a month for your first six months, then $99 a month, which is the standard price. We'll tell you before it changes. On annual billing nothing changes at all — $790 a year, every year. The one thing with a date on it is setup, which is free until November 30 and $499 after that.

Yes. Once your forms are live there's no cap on how many you publish or how many patients complete them.

Send it over and we'll build it: additional or new custom forms beyond setup start at $49 each. Wording changes to forms you already have are free.

No. You pay monthly or yearly and can cancel anytime. If you leave, we'll export your submissions for you.

Yes, with every clinic, before any patient information is collected. It's included in the price.

No. They open a link in the browser they already have, on a phone, a tablet or a computer, at your own web address.
