Oraform vs Jotform for HIPAA patient forms
What is different
Which plan signs. Jotform signs a business associate agreement from its Gold plan up. Oraform signs one on the only plan it has, before a single patient answer is collected — so the plan you compare on price is the plan you are actually allowed to use.
What gets metered. Gold is a single-user plan with 100 forms and 10,000 submissions a month. Oraform does not meter forms, submissions or staff logins, and charges nothing per user.
What it was built to do. Jotform is a form builder for everybody. Oraform only does clinic paperwork, which is why the product has save and resume with patient reminders, QR check-in and link access to the same form, safety alerts on answers, and drop-off figures per question.

Built for clinics
These are not rows in the table above, and that is deliberate: they are things Oraform does, not claims about what Jotform does not. We have only checked Jotform against its own published pricing, which says nothing either way about most of them.
- Save and resume, with reminders. A patient who stops halfway gets an email picking up where they left off, instead of starting the packet again in your waiting room.
- QR check-in, or a link. The same form, reached by scanning a code at the front desk or by following a link sent ahead of the visit.
- Safety alerts on answers. You choose which answers matter — a PHQ-9 item, a score band, a particular yes — and who gets told the moment one comes in.
- Drop-off stats per question. Which question people quit on, so the one quietly costing you patients is a number rather than a hunch.
- Retention set per clinic. How long submissions are kept is yours to set, and it is written into your agreement.
- No per-user fees. Front desk, clinicians and billing all get their own login at no extra cost.
Where Jotform is a better fit
Genuinely, and for more clinics than we would like to admit. If any of these is what you need, Jotform is the better buy and we would rather you knew now.
- You want a template library to start from. Jotform’s is enormous and ours is not — we build your forms instead, which is a different trade.
- You need the form to talk to other software yourself. Jotform connects to far more third-party tools, out of the box, with no conversation required.
- Most of what you collect is not patient information. Event signups, staff surveys, supplier forms, a job application page — one Jotform account covers all of it, and Oraform does none of it.
- You want to sign up and build it tonight. Jotform has a free tier and self-serve checkout. Oraform starts with a call, because we build the forms.
- You are one person and 100 forms is plenty. Then Gold’s single-user limit costs you nothing, and the cheaper tier wins.
Switching is done for you
The reason clinics stay on a tool they have outgrown is the afternoon it would take to rebuild twenty forms. So we do that part.

Does Jotform sign a BAA?
Yes, from its Gold plan upwards, according to Jotform's own published pricing page as last checked on October 13, 2026. HIPAA features are not offered on the tiers below Gold, so there is no BAA to sign on those. Oraform signs a BAA with every clinic on its only plan, before any patient information is collected.
Is Jotform HIPAA compliant on the free plan?
No. On Jotform's published pricing, HIPAA features begin at Gold. A form that collects patient information on a plan with no business associate agreement behind it is a problem no matter how the form itself is built, so the free tier is not a starting point for patient paperwork.
Can we move the forms we already have in Jotform?
Yes, and we do the work. Send us the forms you use today, in whatever format they are in, and we rebuild them in Oraform with every question and every consent carried over word for word. There is no extra charge for rebuilding forms you already have.
Does Oraform charge per user?
No. Front desk, clinicians and billing each get their own login at no extra cost. Jotform's Gold plan, which is the first tier that offers HIPAA features, is a single-user plan on its published pricing.
Is Oraform cheaper than Jotform?
With HIPAA switched on, yes on both billing terms as of October 13, 2026: 79 dollars a month billed annually against 99, and 99 dollars a month billed monthly against about 129. Below Gold, Jotform is cheaper than Oraform and cannot sign you a BAA, which makes those tiers the wrong comparison for patient forms rather than a better deal.
