The service

Oraform provides hosted online forms for healthcare clinics, including form setup, submission storage and export.

Accounts

Clinics are responsible for who on their team has access and for keeping sign-in details secure.

Fees

The service is billed monthly at the price shown on our pricing page. You can cancel at any time, effective at the end of the current billing period.

Patient information

Patient information is handled under the business associate agreement between Oraform and the clinic, which takes precedence over these terms where they differ.

Your responsibilities

Clinics are responsible for the content of their forms, including clinical and legal wording, and for using the service in line with applicable law.

Ending the service

On cancellation, we'll provide an export of your submissions on request, then delete your data as set out in your agreement.
