Who we are

Oraform provides online patient forms to healthcare clinics. This policy covers this website and the Oraform service. Contact: hello@oraform.health.

Patient information

When patients complete forms, we process that information on behalf of the clinic, under a business associate agreement. The clinic controls that information. Patients should contact their clinic about access, correction or deletion.

Information from clinics and visitors

We collect the contact and billing details clinics give us, and basic technical information about visits to this website. We do not sell personal information.

How we protect information

Information is encrypted in transit and at rest, access is restricted and logged, and patient-facing forms carry no advertising or analytics scripts. See our security page.

Retention

We keep clinic data while the clinic is a customer and as set out in their agreement with us, then return or delete it.

We'll post any changes to this policy on this page with a new date.
