What counts as a form?
Any single document a patient fills in: an intake questionnaire, a consent, a screener, a records request. A typical new-patient packet is three to five forms, so setup's ten covers most clinics with room to spare.
What does setup include?
We rebuild up to ten of the forms you use today — whatever format they're in — carrying over every question and consent word for word, and match them to your branding. Free while the launch offer runs, then $299.
Does the price go up after the intro period?
The intro rate covers your first 6 months on monthly billing, or your first year on annual. After that it's the standard $99 a month or $990 a year, and we'll tell you before it changes. Setup stays free either way once you're set up.
Are forms and submissions really unlimited?
Yes. Once your forms are live there's no cap on how many you publish or how many patients complete them.
What if we need an extra form later?
Send it over and we'll build it: additional or new custom forms beyond setup start at $49 each. Wording changes to forms you already have are free.
Is there a contract?
No. You pay monthly or yearly and can cancel anytime. If you leave, we'll export your submissions for you.
Will you sign a BAA?
Yes, with every clinic, before any patient information is collected. It's included in the price.
Do patients need to download anything?
No. They open a link in the browser they already have, on a phone, a tablet or a computer, at your own web address.
